There are over a dozen field types you can add to your questionnaires, including: text fields, number fields, dates, weight/height/gender, checkboxes, radio boxes, tables of text/checkboxes/radio boxes, HTML fields, dropdown menus, long text boxes. You can mix and match them as needed.
You can click a button to send an email (with your branding) to a client. The email will contain custom text and a link to the questionnaire for them to fill in. You can also invite clients to the client portal, where they will be able to log in securely to fill in any questionnaires shared with them. On the other hand, you can also embed a link or web widget in your own website for clients to access the questionnaire. Finally, you can download a blank copy of the form as a PDF file to print and fill in on paper as a last resort.
Yes. For new customers to NutriAdmin, we offer a free service to help you implement your questionnaires. We will do 2 hours of work for you for free - and offer to do more for a small fee if the time exceeds 2 hours. When you join NutriAdmin you will get an email explaining this in detail and you can decide if you want us to help you implement your forms for you.
When you join, you can use default questionnaires provided by us if you want. You can edit them, use them as they are, or delete them if not needed. By default, the following are included: nutritional assessment questionnaire, food diary, food likes/dislikes questionnaire, fitness questionnaire, PARQ form, terms and conditions sample, feedback collection, and more.
NutriAdmin is designed to make the admin work of nutritionists and coaches more efficient. When clients enter data in questionnaires, this is saved automatically to their client records. Then, you have the option to create formatted reports with subsections of the data to provide the clients, doctors, or for yourself. Data is categorized and stored chronologically for you. All the questionnaires are integrated with the rest of the software, so you never have to copy/paste data across systems or rewrite the same information multiple times.
This is a private app clients can log in to when you invite them to the portal. The client portal is secure and private. Clients can log in to fill in questionnaires you have shared with them. Yes. You can duplicate any form and create variations. For example, you could have 8 versions of a food diary to keep track of 8 weeks of eating over time. You can make variations for follow-up notes too. Any information that you want to keep track over time is a suitable candidate for variation forms.
By default NutriAdmin is only provided in English, but you can create questionnaires in any language you wish. Some of our customers have used the software successfully in Spanish, Arabic, German, French, Portuguese, Greek, and many other languages.
