If you have recently joined NutriAdmin, it is possible you already have a series of questionnaires/consultation/case notes you use with your clients to gather important information about them prior to consultations. We find many nutritionists/dietitians have questionnaires in Word or PDF format.
It is possible to create a new questionnaire mimicking your original form(s) in NutriAdmin. The process is not too difficult, and can save you considerable time in the long-run. However, we do not have a way of automatically importing a questionnaire from a document to our web format. This is due to the technical complexity and endless variety of different formats that can be used for questionnaires amongst different practitioners.
We encourage each NutriAdmin user to setup their own questionnaires following the How to create a new questionnaire guide (link above). This process can take between 30 minutes and 2 hours depending on how long and detailed your questionnaires are. This setup operation only needs to be done once, and will start saving you time immediately, as you start sending your questionnaires to clients with one click, and receiving client data to your NutriAdmin account in a tidy and efficient way.
The above said, we understand setting up your questionnaire can be a tedious process. For this reason, we offer the option to setup questionnaires for users here at NutriAdmin in exchange for a one-off fee of USD$10/hour worked. If want to setup your questionnaires to match one of your existing documents, you can email us the form and we will set it up for you – saving you the hassle of doing it yourself.
This service is entirely optional, you can always setup your questionnaires on your own for free. If you email us the forms we can give you a quote of how much they would cost to implement from our end.
If you want us to set your questionnaires up for you, all you have to do is to email team@nutriadmin.com attaching your questionnaires in Word, PDF, or similar format, and we will give you an approximate quote of how much time we expect the task to take. We can complete most questionnaire setups in 30-120 minutes (depending on the length and complexity of your form), so you can expect a cost in the range of USD$5-USD$20 for most cases per form.
We are also able to charge in other currencies if you signed up to NutriAdmin using GBP, EUR, NZD, CAD, or AUD. If you want to change your currency, please check this tutorial before submitting your questionnaire creation request.
Upon receiving a quote, you can decide whether you are interested or not. We will not charge you any money for setting your questionnaire up unless you approve this expense first. On the other hand, we also reserve the right at NutriAdmin to reject a questionnaire setup task if there are major circumstances preventing us from accepting the service request.
As soon as you confirm you would like us to set your form for you, and you approve our quote, we will start work in configuring your questionnaire in NutriAdmin. Typically, we are able to set most questionnaires up within 1-3 working day – although this time could stretch in busy activity periods or if the questionnaires are very long.
Our team is experienced and has a track record of performing these kind of questionnaire configuration tasks successfully. If we fail to deliver your questionnaire on time, or if the result is not acceptable, you are entitled to request a refund for your questionnaire creation service expense.
We will bill your NutriAdmin registered card shortly after creating the questionnaire for you. You will receive a receipt via email, and the transaction will show up on your bank statement as either NUTRIADMIN SERVICES, NUTRIADMIN or in some cases MAGOSOFT LTD (which is our company name).
As a final note, we would like to offer this service for free. However, due to the high volume of requests we have received in the past, we have no other option than to charge a fee for this service in order to make these tasks compatible with running and improving NutriAdmin. We have worked on efficient processes from our end so that we are able to offer this service as cheaply as possible.
